⚠ Failure mode — uterine inversion
The most dangerous error in this procedure is applying controlled cord traction without counter-pressure, or pulling on the cord alone. This can turn the uterus inside out — a life-threatening obstetric emergency.
Do it right: place the guarding hand suprapubically and press the uterus up and back, apply steady, gentle traction only with a contraction, and always work from the perineum — never the head.
