Here is a structured course outline for "TEST Hand Hygiene Fundamentals," designed for Community Health Officers in Nigeria, aligning with the brief and RCI instructional design principles. --- ## Course Outline: TEST Hand Hygiene Fundamentals **Audience:** Community Health Officers (CHOs) **CPD Hours:** 1 **References:** WHO Guidelines on Hand Hygiene in Health Care --- ### **Course Overview:** This essential CPD course equips Community Health Officers with the fundamental knowledge and practical skills required for effective hand hygiene. Focusing on WHO guidelines, participants will learn *when* and *how* to perform hand hygiene using both soap and water and alcohol-based hand rubs, crucial for preventing healthcare-associated infections and promoting community health in diverse Nigerian settings. --- ### **Learning Objectives:** By the end of this course, participants will be able to: 1. **Recall** the critical importance of hand hygiene in preventing infection transmission in healthcare and community settings. 2. **Identify** the WHO's "5 Moments for Hand Hygiene" in various clinical and community-based scenarios. 3. **Perform** (demonstrate understanding of the steps for) the correct technique for handwashing with soap and water according to WHO guidelines. 4. **Perform** (demonstrate understanding of the steps for) the correct technique for hand rubbing with an alcohol-based hand rub (ABHR) according to WHO guidelines. 5. **Apply** effective hand hygiene practices consistently to enhance patient safety, personal protection, and community health. --- ### **Modules:** This course is divided into 4 modules, each designed for approximately 10-15 minutes of engagement, allowing for an introduction, conclusion, and assessment within the 1-hour CPD timeframe. --- #### **Module 1: The Critical Importance of Hand Hygiene** * **Description:** This module establishes the foundational understanding of why hand hygiene is an indispensable practice for Community Health Officers, highlighting its role in breaking the chain of infection and safeguarding public health. * **Key Teaching Points:** * Definition and purpose of hand hygiene. * The burden of Healthcare-Associated Infections (HAIs) and community-acquired infections (e.g., diarrhoeal diseases, respiratory infections) in Nigeria. * Understanding the Chain of Infection and how hands act as vectors for pathogen transmission. * Benefits of effective hand hygiene: enhanced patient safety, protection of healthcare workers, and improved community health outcomes. * The unique relevance and challenges for Community Health Officers in diverse Nigerian settings (e.g., resource limitations, mobile clinics, home visits). --- #### **Module 2: When to Clean Your Hands: The WHO 5 Moments for Hand Hygiene** * **Description:** This module focuses on the strategic timing of hand hygiene, introducing and explaining the internationally recognized "5 Moments" framework for preventing infection transmission. * **Key Teaching Points:** * Introduction to the WHO "5 Moments for Hand Hygiene" as a global standard. * Detailed explanation and practical, context-specific examples for each of the 5 moments relevant to CHO practice: * **Moment 1: Before touching a patient** (e.g., before taking vital signs, physical examination during a home visit). * **Moment 2: Before a clean/aseptic procedure** (e.g., before wound dressing, administering injections, catheter care). * **Moment 3: After body fluid exposure risk** (e.g., after handling blood, urine, sputum, changing soiled linen). * **Moment 4: After touching a patient** (e.g., after completing a physical exam, assisting a patient with mobility). * **Moment 5: After touching patient surroundings** (e.g., after touching a bed rail, medical equipment, or a patient's personal items). * The rationale behind each moment and its contribution to infection prevention. --- #### **Module 3: How to Clean Your Hands: Handwashing with Soap and Water** * **Description:** This module provides step-by-step guidance on the correct technique for handwashing with soap and water, emphasizing its use when hands are visibly soiled or when caring for patients with specific infections. * **Key Teaching Points:** * Indications for handwashing with soap and water (visibly soiled hands, after using the toilet, after contact with *Clostridioides difficile*). * Necessary supplies: Access to clean running water, adequate soap (preferably liquid), and a clean, single-use towel or air dryer. * Detailed, WHO-recommended 6-step handwashing technique (illustrated with clear visuals/video demonstration): * Wet hands, apply sufficient soap. * Rub palm to palm. * Right palm over left dorsum with interlaced fingers, and vice versa. * Palm to palm with fingers interlaced. * Backs of fingers to opposing palms with fingers interlocked. * Rotational rubbing of left thumb clasped in right palm, and vice versa. * Rotational rubbing, backward and forwards with clasped fingers of right hand in left palm, and vice versa. * Recommended duration: Minimum 40-60 seconds. * Importance of thorough rinsing and drying to prevent re-contamination. --- #### **Module 4: How to Clean Your Hands: Alcohol-Based Hand Rub (ABHR) Technique** * **Description:** This module instructs on the proper and effective use of alcohol-based hand rubs, highlighting their advantages and correct application as the preferred method for routine hand antisepsis. * **Key Teaching Points:** * Indications for ABHR use (hands not visibly soiled, before and after most patient contacts). * Advantages of ABHRs: Speed, effectiveness, accessibility, and improved skin tolerance compared to frequent soap and water washing. * Necessary supplies: Availability of WHO-recommended alcohol-based hand rub formulation (gel, foam, or liquid). * Detailed, WHO-recommended 6-step hand rub technique (illustrated with clear visuals/video demonstration): * Apply a palmful of product to cover all hand surfaces. * Recommended duration: 20-30 seconds, or until hands are completely dry. * Important considerations: Using an adequate amount, ensuring all hand surfaces are covered, and allowing hands to air dry completely. --- ### **End-of-Course Assessment:** To successfully complete the course and receive CPD certification, participants must achieve a minimum pass mark of **80%** on the following assessment: * **Format:** Multiple-Choice Questions (MCQs) and Scenario-Based Questions. * **Number of Questions:** 10-15 questions. * **Content Alignment:** * Questions will assess the understanding of the importance of hand hygiene. * Scenarios will require participants to identify the correct WHO 5 Moments for Hand Hygiene. * Questions will test knowledge of the step-by-step procedures for both handwashing with soap and water and alcohol-based hand rub, including critical timings and indications. * Questions will assess the ability to apply hand hygiene principles to practical community health settings. Upon successful completion of the assessment, participants will receive their 1-hour CPD certificate. ---
